REGISTRATION REQUIREMENTS
- Immunization Form (Form 3231) signed by the health department or a licensed Georgia physician.
- Certificate of ear, eye, and dental screening (Georgia Department of Human Resources Form 3300) signed by the health department or a licensed Georgia physician.
- Government-issued birth certificate. According to Georgia law, children must be 5 years old on or before September 1 in order to enroll in Kindergarten.
- Child’s Social Security card (original)
- Proof of residency. To enroll at Sarah Smith Elementary School , the student’s parent or legal guardian must be a resident in the attendance zone of the school. Appropriate records for verification must include a the most recent Georgia Power bill (original), and one other document from the following:
- Home mortgage payment book
- Apartment lease showing name of legal guardian/parent
- Homeowner’s insurance
- Current paycheck stub
- Current bank statement
- Driver’s license showing legal guardian/parent’s current address with one of the above items
- Copy of sales contract (if house is under construction or being purchased)
- Receipt to have utilities connected
- Academic/Special Needs records, if applicable.
